Founded as part of the Vermont Blueprint for Health, the Community Health Team works with community members, patients, and their families and caregivers to support self-management of chronic conditions. The team is a resource for patients needing help getting their health on track, works to reduce barriers to care and provides short-term mental health and substance abuse counseling.
Care coordinators and behavioral health specialists work directly with patients to determine their needs and to help them access community supports, such as state and local agencies, mental health agencies for longer term counseling, health self-management support and education, and financial assistance.
The Blueprint team also leads many classes and support groups, including Healthier Living Workshops, one-on-one and group tobacco cessation education, certification and non-certification CPR classes, and chronic illness and caregiver support groups.
