 Founded as part of the Vermont Blueprint for Health, the Community Health Team works with community members, patients, and their families and caregivers to support self-management of chronic conditions. The team is a resource for patients needing help getting their health on track and works to reduce barriers to care.
A Gifford care coordinator works directly with patients to determine their needs and to help them access community supports, such as state and local agencies, behavioral health support, health self-management support and education, and financial assistance.
