Care coordinators help patients overcome socioeconomic and other barriers to better self-manage chronic conditions. Behavioral health specialists work with your health care provider to offer short-term mental health help for both children and adults, as well as short-term substance abuse counseling. Conditions treated include pediatric and adolescent issues, chronic pain, insomnia, anxiety and bereavement.

Mental health
Outpatient mental health services from a licensed psychologist are available by appointment in a relaxing office space on the upper floor of the health center.

Where to find us
Rehabilitation services are located on the ground level of the health center. The Diabetes Clinic, Blueprint team, mental health practitioner and a private practice dentist office are on the upper level.
