For patients taking ongoing medication, your health care provider will want to see you at regular intervals to monitor your health. Our goal is to provide you with a prescription at the time of your visit that will last until we see you again. For patients needing prescription refills between appointments, we offer a prescription refill line.
To fill your prescription, we will need the following information:

Your full name

Your phone number

Your date of birth

Your health care provider’s name

The name and strength of the medication
-
The directions for how you take the medicine (i.e. once daily at bedtime)

The pharmacy that you use
Your refill request will be completed within 48 hours. Seven days advance notice is required for mail order prescriptions and all medications requiring pre-authorization by your insurance company.
