Social Services
The social worker on Gifford’s staff provides services to individuals and families whose physical or emotional functioning has been impacted due to illness, hospitalization or long-term care placement. Services may include information, referral and advocacy to accessing community-based health and social services, advance directives planning, financial problem solving, and short/long term counseling.
To make a patient’s return home as easy as possible and to ensure for on-going care if needed, Gifford’s Social Services department offers discharge planning, as well as guidelines on matters such as sourcing of adaptive medical equipment and referral to appropriate community and agency support systems.
